Medical Records

St. Clair Health offers several convenient ways for you to obtain a copy of your medical records.

  1. Patient Portal: St. Clair Health’s patient portal is called Follow My Health. Patients can have immediate access to their records. Click here for more information on Follow My Health.
  2. NEW! Request electronically through the patient request portal: Patients can request their records electronically. Click here to access the request portal.
  3. Print out a copy of our release of information form: Fill out the form completely and sign. Click here to access the authorization form.

    Once completed, the form can be sent to our Release of Information Department via mail, email, or fax (see below):

If you would like to stop by in person, the Medical Record Department is located on the 4th Floor of the Hospital.

The fee for copies of records is based upon the fee schedule set by the Commonwealth of Pennsylvania. You may contact the Medical Record Department at 412.942.1102 for more information or with questions.

Monday through Friday, 8:00 am to 4:00 pm (closed on holidays)
