Medical

St. Clair Hospital offers employees medical insurance through a choice of carriers including Highmark, UPMC Health Plan (narrow network), or United Health Group (UMR). All medical plans include prescription drug coverage through our PBM provider. Spousal coverage is available for those who do not have employer group health coverage available. If completed timely, your benefit effective date is the 1st day of the month after date of hire. If not completed prior to original benefit start date, benefits will begin first of the following month.

Dental

Dental insurance is offered through either the United Concordia – Core Program or the United Concordia – Enhanced Program, which gives eligible employees access to a national dental network. Two programs are available to select from. A CORE Program that is more economical, however, has a small deductible associated with the employee only plan or the family coverage plan. The Enhanced Program has no deductible and offers an orthodontic benefit with no age limit. If completed timely, your benefit effective date is the 1st day of the month after date of hire. If not completed prior to original benefit start date, benefits will begin first of the following month.

Vision

St. Clair Hospital offers vision insurance through either VSP (narrow network) or Highmark Fashion Focus (Davis Vision broad network). A distinguishing factor of St. Clair’s vision plan is its annual exam and corrective vision eyewear benefit. Reduced, out-of-network coverage is also provided. 

Life Insurance

All full and part-time employees are covered with life insurance equal to one times their annual salary (rounded to the nearest thousand), as well as accidental death and dismemberment (AD&D) insurance. Additional life insurance is available via payroll deduction in volumes of one, two or three times an employee’s annual salary. Dependent life insurance is also available upon purchase of supplemental life insurance.

Short-Term Disability Insurance – Part-Time Employee

Voluntary Short term disability insurance is available through VOYA and is effective on the first day of the month following 90 days of service.  Part-Time employees are offered three options for short term disability coverage. Based upon the coverage that the employee elects, the employee will be entitled to a predetermined weekly income replacement benefit.  Short-term disability insurance may be payable for illness or injuries excluding on-the-job injuries which are covered by workers’ compensation. Also offered by VOYA is Accident, Critical Illness and Hospital Indemnity plans.

Short-Term Disability Insurance – Full-Time Employee

Short term disability voluntary insurance is available through VOYA and is effective on the first day of the month following 90 days of service. St. Clair Health pays 100% of the Core Coverage Plan that covers 50% of your weekly base salary. As an option, you may choose to purchase the Buy-Up Plan and increase your coverage from 50% to 60% of your weekly salary. Short Term Disability begins on the 15th day of a continuous non-work-related injury or illness. Also offered by VOYA is Accident, Critical Illness and Hospital Indemnity plans.

Long-Term Disability Insurance

Long term disability insurance is provided to all full time employees on January 1st following date of hire and is insured through Lincoln Financial Group.  The plan may provide monthly benefit payments for long term absence due to illness or injury after a six-month elimination period.  The benefit payment is 60% of an employee’s base monthly earnings subject to plan maximum benefits.
