Your gift builds on the 60-year tradition of community support that has made St. Clair Hospital a premier regional healthcare provider.

When making your gift, please select from one of the following:

• Patient Assistance 
   Helps eligible patients to pay for home care services, prescription drugs, personal care home stays, medical equipment and other medically necessary items.  
• Patient Transportation
  A free service available to local residents who require rides to and from the Hospital and its affiliate locations. The Patient Transportation service maintains a fleet of eight vehicles, employs six full-time drivers, and makes approximately 13,000 trips annually.
• Unrestricted Annual Fund
  Gifts are directed to programs/projects that are current Hospital priorities.
• Gift of Gratitude
  Is there a member of the Hospital staff that you would like to recognize with your contribution? If so, simply include their name (along with your gift designation) under "Purpose" on the next page and we will notify the person.

Please remember to write-in one of the gift designations above. Gifts without a specified designation will be directed to our unrestricted fund. Gifts designated to programs/areas other than those listed above will be directed to existing funds deemed to be the closest appropriate match to the donor's intent.
