St. Clair Hospital offers employees medical insurance through a choice of carriers including Highmark (narrow network), UPMC Health Plan (narrow network) or United Health Group (UMR broad network). All medical plans include prescription drug coverage and may include spousal coverage for those who do not have group coverage available. Coverage is effective the first day of the month following 30 days of service.
Dental insurance is offered through either the MetLife network or Highmark (United Concordia Advantage Plus network), which gives eligible employees access to a national dental network. St. Clair Hospital’s dental plan is unique, with no annual deductible and an extensive list of providers. Coverage is effective the first day of the month following 30 days of service.
St. Clair Hospital offers vision insurance through either VSP (narrow network) or Highmark Fashion Focus (Davis Vision broad network). A distinguishing factor of St. Clair’s vision plan is its annual exam and corrective vision eyewear benefit. Reduced, out-of-network coverage is also provided.
All full and part-time employees are covered with life insurance equal to one times their annual salary (rounded to the nearest thousand), as well as accidental death and dismemberment (AD&D) insurance. Additional life insurance is available via payroll deduction in volumes of one, two or three times an employee’s annual salary. Dependent life insurance is also available upon purchase of supplemental life insurance.
Short-Term Disability Insurance
Short-term disability insurance, which is available through AFLAC, pays a percentage of an employee’s salary if he or she becomes temporarily disabled (unable to work for a short period of time due to sickness or injury, excluding on-the-job injuries, which are covered by workers compensation insurance). The short-term disability policy provides employees with a weekly portion of their salaries for a predetermined amount of time, based upon the plans employees purchase.
Long-Term Disability Insurance
Long-term disability insurance is provided to all full-time employees on January 1, following an employee’s date of hire. The plan stipulates that a six-month elimination period must be met, prior to any benefits being paid. Benefits are payable at 60% of an employee’s base monthly earnings.
